Participant (\u201cParticipant\u201d, \u201cyou\u201d or \u201cyour\u201d) agrees to be bound by these Auto Renew Terms and Consent to Membership Agreement (\u201cTerms\u201d) with respect to all services provided by Club Fitness (\u201cServices\u201d). All references to \u201cClub Fitness\u201d in these Terms mean Club Fitness, Inc., its parents, affiliates or subsidiaries (\u201cClub Fitness\u201d or \u201cwe\u201d or \u201cus\u201d) providing Participant the Services, as well as third parties Club Fitness may retain to provide the Services.<\/span><\/p>\n <\/p>\n By checking the appropriate box, you give your express, informed consent and agree to pay the applicable dues, as well as the increased dues amount and annual membership fee, any new, additional or increased taxes imposed as set forth herein, and any past due amount owed, by automatic payments billed to a checking, saving, credit or debit card account, as identified in the attached Exhibit A. Once the <\/span>automatic deductions begin you will continue to be billed until a 30-day notice of your intent to terminate this Agreement is received as set forth herein. Upon completion of your installment payment(s) your membership shall automatically renew until you terminate your membership in accordance with these Terms. <\/b>Your last billing will occur within 30 days of your termination notice being received, as well as being billed for all past dues amount owed.\u00a0<\/span><\/p>\n <\/p>\n MEMBER\u2019S RIGHT TO CANCEL: <\/b>You may cancel, your Membership Agreement online, in writing by certified mail, return receipt requested, to Club Fitness 7055 Mexico Road Box 1210, St. Peters, MO 63376 or by<\/span> personally visiting any Club Fitness location during business hours and informing the staff of your intention to terminate your Membership Agreement. Any notice of termination submitted in person shall not be valid unless you receive electronic or written receipt <\/span>and<\/b> confirmation of termination. Please retain your electronic or written receipt and confirmation of termination for your records. A thirty (30) day notice period is required for termination of your Membership Agreement. You are required to pay the annual membership fee, as set forth in Exhibit A, if it falls within this thirty-day notice period.\u00a0 You will also be responsible for any past due amounts owed.
